Drivers Permit & License Information

Documents required for 15-year-old permit:

– Original or state-issued Birth Certificate.

– Original Social Security Card or printout from Social Security Administration.

– Completed and notarized Application for Mississippi Drivers License with signature 

   of single parent or guardian or both parents or guardians.  If parents are divorced, 

   the parent with custody must sign.  Parents MUST sign in front of the notary. 

– Current School Attendance Form.  Must be obtained within 30 days of applying for 

   permit and is not valid after 30 days from date on form.  This form must be requested 

   by email, and students have received emails regarding this. 

– Two current proofs of residency, no more than 30 days old. 

– Cost of learner’s permit $7. 


Documents required for 14-year-old permit:

– Student MUST be enrolled in Drivers Ed at Houston High School.

– All documents listed above for 15-year-old permit. 

– Form signed by Principal and Drivers Ed Teacher stating the student is 14 years old. 


Documents required for Drivers License:

–  Original or state-issued Birth Certificate.

–  Original Social Security Card or printout from Social Security Administration.

– Completed and notarized Application for Mississippi Drivers License with signature 

   of single parent or guardian or both parents or guardians.  If parents are divorced, 

   the parent with custody must sign.  Parents MUST sign in front of the notary. 

– Current School Attendance Form.  Must be obtained within 30 days of getting drivers 

   license and is not valid after 30 days from date on form.  This form must be requested   

   by email, and instructions have been emailed to students regarding this. 

– Two current proofs of residency, no more than 30 days old. 

– Cost of drivers license $24.



Additional information regarding permit and drivers license:

– Drivers Ed Teacher holds 14-year-old permit until student turns 15, at which time the 

   student takes the 14-year-old permit and all documents required for 15-year-old 

   permit to the DMV to obtain a 15-year-old permit.  The permit test will not have to be 

   retaken.  

– If student is 17 and has not gotten permit or license, both of these may be obtained at 

   the same time with the required documents for 15-year-old permit.

– If you will be 18 when you get your permit or drivers license, an Attendance Form is 

   NOT required.  

– Receipt from DMV (original or copy) for permit or drivers license can be used 

   to excuse the student when a day is missed for the purpose of obtaining permit or 

   license.  Receipt must be brought in within five days from date of going to DMV.  

– Students have received numerous emails from Mrs Jan Hollingsworth regarding how 

   to get an Attendance Form.  The form will not be issued if students do not follow these   

   instructions. 

– This is the Attendance Form email summarized:  Students must email Mrs Jan 

   requesting an attendance form and include their FULL LEGAL GIVEN NAME AS IT 

   APPEARS ON THEIR BIRTH CERTIFICATE and CURRENT ADDRESS.  The form must be 

   requested several days before the student plans to go to the DMV because the forms 

   are done once daily, first thing in the morning before 8am.  There are NO exceptions, 

   especially due to lack of preparation and planning .  The attendance forms expire 30 

   days from the date on the form.
